We take the health of our students very seriously and so we ask that all medical conditions regarding your child be relayed to the school. Any health care plans that we should be made aware of must be given to the office for photocopying and kept up to date at all times.

Below are a some *general care plans that you can complete and return to the office should they be relevant to your child’s illness. *Please note all plans should have a health care professional approve the plan, and some plans may not be appropriate to a particular individual so may need an alternative health care plan.
